The buyer is seeking a contractor to provide support to York Place, to drive the planning and implementation of improvements identified in a recently-undertaken discharge stocktake. Additionally, the system would like to review its Pathway 3 policy and process - looking across the whole patient journey, with a view to improving long term outcomes for people leaving hospital on this pathway. Additional information: The contracting authority considers that this contract may be suitable for economic operators that are small or medium enterprises (SMEs).
