Description

This is a request for quotation exercise. NHS West Yorkshire Integrated Care Board (Calderdale Cares Partnership) is seeking a provider to provide intermediate care residential beds provision for the winter period 2023/24. A provision of 5 intermediate care residential beds for patients in Calderdale. The purpose of the service is to provide flexible residential beds within a community setting for adults who do not need to be in hospital, but who need a period of assessment and rehabilitation before returning to their usual place of residence. The service will have a strong rehabilitation focus, with individuals being supported to regain their optimal level of functional independence and to self-care wherever possible. The therapy service for Intermediate Care in Calderdale is provided by Calderdale and Huddersfield NHS Foundation Trust. This is a dedicated team of therapy professionals who work collaboratively and with a multi-disciplinary approach with the care home provider and wider health and social care colleagues to deliver rehabilitation programmes to service users whilst residing in Intermediate Care.
