NHS Lincolnshire Integrated Care Board is inviting suitably qualified and experienced providers to express interest in delivering the Service. The key objectives of the Procurement are to establish providers who can host Social Prescribing Link Workers to provide a proactive, preventative approach to social prescribing that will identify patients who would benefit most from the support. Social prescribing link worker (s) will work as part of a Primary Care Network or part of the Locality Mental Health Team and will collaborate with colleagues as part of a wider multi-disciplinary team.
