NHS North West London Integrated Care Board (the ICB) requires the provision of self-care & support for self-management services for the local population in RBKC, Queens Park & Paddington, to improve the health & wellbeing by promoting & providing opportunities for increased access to self-care activities, starting 1st April 2023 to 31st June 2023. The service is currently being delivered by Kensington & Chelsea Social Council (KCSC). The ICB is under the impression that KCSC is the only capable provider to successfully deliver this service for the proposed 3 months within the total budget of £60,000. The ICB seeks to be transparent in it's intention to award the contract to KCSC. This service works closely with the Case Managers & Health & Social Care Assistants to provide services to patients, within a voluntary & community sector setting. Further aims are: • To improve the health & wellbeing of the residents in the West London catchment area (RBKC, Queens Park & Paddington) • To promote the benefits of a healthy lifestyle • Increase access to third sector provision to promote overall wellness • Provide opportunities for people with underlying medical conditions to become more active • Equip patients with the knowledge & skills to become better managed with their long term conditions • To promote long-term behaviour change Capable providers that meet the below conditions for participation may respond to this opportunity: 1. For the service to be fully functional by 1st April 2023 in the required area. 2. The provider to demonstrate existing, well developed, relationships with local community & voluntary organisations, GP practices & Primary Care Networks, other NHS providers offering services in the area. 3. The provider to demonstrate a good understanding of the needs of local communities, & evidence of undertaking collaborative engagement with local people. The service continues to develop learning & development whilst ensuring: • Identify self-care provision/activities in the 3rd Sector • Maintain a list of potential self-care providers together with the detail of service provision • Provide a co-ordination service which will supply a list of services available, provide a payment mechanism for the services & will monitor uptake • Commission 3rd Sector organisations to provide self-care services through a mix of block & spot purchasing • Manage & monitor the service uptake & achievement of goals. The ICB is committed to assess the needs of the service users & consider how (if relevant) it needs to secure the needs of the people who use the services, & improve the quality & efficiency in the provision of the services including through the services being provided in an integrated way. Capable providers may come forward by 17:00 on 24th February 2023.
