Description

The provision of extra care offers the opportunity for residents to live within their own home, therefore minimising the need to go into a residential care setting, which can sometimes be presented as the only option available and not always of the choice of the individual or the best option in maintaining independence. The purpose of extra care is to deliver quality care, which is enabling, and outcome focused to support tenants to live as independent lives as possible. This will include frail and older people, those with dementia, adults with learning disabilities, adults with mental ill health and adults with a physical and/or sensory disability. The service aims to help those adults with assessed eligible need, remain in their own home by providing essential personal and practical support to maintain or enable independent living skills and achieve the outcomes which matter to them.
