NHS HMR CCG and Rochdale Borough Council are proposing a new integrated service model for children's community health services that will support our drive to improve outcomes for children and young people. The service will operate as a single entity, with each individual service element contributing to and supporting delivery of a shared set of outcomes. The service delivery model will build upon the existing integrated children's acute and ongoing needs service (CAONS) and ensure that prevention and early help health services are also included. The service model at this stage will include the following existing services: • Children's acute and ongoing needs (CAONS) • 0-5 universal public health services (Health Visiting) • 5-19 universal public health services (School Nursing) The services are to be delivered as part of a comprehensive 0-19 years (0-25 years to meet the CCG's statutory obligations regarding specialist educational needs and disability (SEND)) seamless service for children and young people. The Contractor will understand the health needs of this population and through a process of assessment, planning and support, will improve the health and wellbeing outcomes for all children and young people, thereby helping them to achieve their potential and laying the foundations for future health and well-being in adult life. The CAONS is a well-established co-produced integrated children's community health model and this commission will look to build upon, maintain and support innovation. The contractor will be expected to contribute to the continued development of the Family Services Model (FSM) to deliver a whole system approach for children, young people and families that support existing integrated delivery and develops integrated pathways and offer from universal / community level support to highly specialised and acute interventions.
