Development a local integration of the population Health web-based mapping business intelligence software platform, SHAPE Local, for the purpose of profiling and service planning for Places (East Riding of Yorkshire; Hull; North-East Lincolnshire; North Lincolnshire; North Yorkshire; and York), Primary Care Networks (PCNs) and Integrated Neighbourhood Teams (INTs). Segmentation visualisations - creation of visual indicators; and integration with the NHS Humber and Yorkshire ICB data warehouse - the use of SHAPE to help identify neighbourhoods and local area profiles, including drawing from the Humber and North Yorkshire segmentation data and utilising the tools available within SHAPE such as population targets The service provision will include analyst and development time and training sessions of the use of the software.
